
Covid-19/Respiratory Infection Measures
In order to best protect our community and staff members, we have implemented several risk reduction and social distancing measures:

You will be asked a few questions when booking and checking in for your appointment according to the RACGP triage and screening standards.

If you have cold and flu symptoms, you can organize a telephone appointment initially. For in person appointments, please organize a COVID-19 swab, or a RAT test on the day of your appointment. Patients with a negative COVID-19 test can attend the practice but we require a mask to be worn to minimise illness spreading to your doctor and other patients.

Telephone consultations are available. Patients with a medicare card who have seen one of our doctors for a face-to-face appointment in the last 12 months, or are under the age of 12 months of age or homeless can access a Medicare rebated phone consultations. For further information you can speak to one of our friendly receptionist staff.

For new patients, please arrive 10 minutes prior to your appointment in order to complete the new patient form and allow our reception staff to complete your patient file.

We do not prescribe drugs of dependence such as a Diazepam, Endone, Panadeine forte, Tramadol or Oxazepam for new patients. Please be aware that doctors may not prescribe drugs of dependence.

Fees
We are a mixed billing practice. A $30 out-of-pocket fee applies to standard GP consultations, and all accounts must be paid in full on the day of your appointment.
​
Commonwealth Concession Card holders and children under the age of 16 are eligible for bulk billing, except on weekends and public holidays, when a $30 gap fee applies. The Medicare rebate can be processed and refunded immediately to your EFTPOS or debit card—please speak to our reception staff if you need assistance with this.
​
Telephone consultations also incur a gap payment. A Medicare rebate is available only if you have had a face-to-face consultation at our clinic within the past 12 months. If you are booking a telephone appointment online, please be aware that it is your responsibility to ensure you meet this eligibility requirement, as we are unable to check your last face-to-face appointment date on your behalf.
Some appointments are fully bulk billed, including Health Assessments such as the 45–49 Year Old Health Check and the Over 75 Health Check, as well as Chronic Disease Management Plans and Reviews.

If your newborn is awaiting a Medicare card, full payment is required on the day of the appointment. A receipt will be provided so parents can claim the rebate from Medicare once the card is issued.

PROCEDURES
​
We offer Mirena insertions, iron infusions, and minor procedures or skin excisions and circumcisions. These fees vary depending on your treatment plan and must be paid in full on the day of your appointment. We recommend speaking with your doctor or our reception staff in advance for detailed information about any applicable charges.

HOTDOC PREPAYMENT ​
​
Klemzig Village Medical Clinic is now offering online payments through HotDoc for certain appointment types, including Telehealth and services for patients without a Medicare card. Before your appointment, you will receive a message from HotDoc requesting your payment details. Rest assured, no payment will be processed until after you have spoken with or seen your doctor.

Newborn and Child vaccinations NIP
​
We offer childhood vaccinations under the National Immunisation Program (NIP). Please call the clinic directly to ensure the correct appointment is booked with both a doctor and nurse, as these are long appointments and cannot be booked online.
​
Please note that we do not provide 4–5 Year Old Health and Development Checks. The Child and Family Health Service (CaFHS) provides these checks for all 4–5 year olds at local CaFHS clinics.

Transfer of notes
​
To transfer your medical record from a previous clinic to Klemzig Village Medical Clinic, please fill out and sign this form. Once completed and signed it can be faxed or dropped off to the reception staff at Klemzig Village Medical Clinic. We can arrange a summary for no charge.

Telephone policy
​
We understand there may be times you wish to speak to your GP for further advice, however, our doctors are unable to take non-scheduled phone calls. We can offer you a scheduled phone appointment with your GP to discuss your needs. Please inform our reception staff if your call is urgent. If your medical concern is an emergency, please call an ambulance on 000.

Email policy
​
The RACGP does not recommend the use of email for the transfer of patient information. Email is not considered secure as it is not encrypted. We regret we are generally unable to send your results and other clinical information to you by email. If you would like copies of your results we are happy to provide you with a printed copy.

Reminders
​
Our Practice is committed to preventative care. We use a computerised reminder system for specific recalls, such as reminders for follow up blood tests and vaccinations.
​

Confidentiality
​
Your medical file is a confidential document. It is the policy of this Practice to maintain security and confidentiality of personal health information at all times and to ensure that this information is only available to authorised members of staff.
​
Vaccinations
All government-funded vaccines are available free of charge to those who meet the eligibility criteria and hold a valid Medicare card.
Please note: standard appointment fees apply. For patients without a Medicare card, Klemzig Village Medical Clinic does not provide catch-up vaccinations.

We are unable to update the Australian Immunisation Register (AIR) for vaccinations not administered at our clinic. If you need assistance with AIR, please: Call 1800 653 809 or Contact your local council for information about your nearest vaccination centre
​
If you have had immunisations overseas, please visit a community or council immunisation health centre in your local council area for advice and support. If your vaccination documents are not in English, you can have them translated there.

Travel vaccination advice is available. A script for any recommended vaccines can be provided during your appointment.
Anti-social Behaviour Policy
​
We understand that you may be unwell, stressed or anxious. Our Practice has a zero-tolerance policy towards violence, anti-social behaviour and will not tolerate verbal or physical threats made towards members of the staff or General Practitioners or other patients.
​
DNA POLICY
Failure to attend appointments or give 2 hours cancellation notice will result in a $25 DNA fee. Please avoid this by calling ahead if you cannot make your appointment. This fee must be paid as soon as possible, and before your next appointment is booked. Allied Health required 24-48 hours notice and have higher DNA fees.

Workcover or MVA consultations
​
If you wish to have a WorkCover or MVA consultations please inform our reception staff at the time of making the appointment and fill out the relevant form prior to your appointment with the GP. The account will need to be paid privately on the day, we will issue an invoice which you can take to your MVA or work cover claims manager.

WorkCover and Motor Vehicle Accident accounts are ultimately the patient’s responsibility. Please ensure your WorkCover claim is submitted and notify us of your claim number. For Motor Vehicle Accident claims, please identify and advise us of your Compulsory Third Party insurer as soon as possible – details on how to find your insurer can be found at http://www.ctp.sa.gov.au/insurers.html
