You will be asked a few questions when booking and checking in for your appointment according to the RACGP triage and screening standards.
If you have cold and flu symptoms, we ask you do not attend the practice and organize a telephone appointment initially. Patients who have seen the GP previously, attended the practice previously, under the age of 12, homeless or in a COVID hot spot can access Medicare funded phone consultations. For further information you can speak to one of our friendly receptionist staff.

We welcome all patients to our medical clinic.
For new patients, please arrive 10 minutes prior to your appointment in order to complete the new patient form and allow our reception staff to complete your patient file.

We do not prescribe drugs of dependence such as a Diazepam, Endone, Panadeine forte, Tramadol or Oxazepam for new patients.
We are a bulk billing practice, all patients with a Valid Medicare card will be bulk billed. Non-Medicare care holders will be charged a full fee on the day.
Certain procedures and treatments do incur a private out of pocket cost.

Transfer of notes
To transfer your medical record from a previous clinic to Klemzig Village Medical Clinic, please fill out and sign this form. Once completed and signed it can be faxed or dropped off to the reception staff at Klemzig Village Medical Clinic.
We understand there may be times you wish to speak to your GP for further advice, however, our doctors are unable to take non-scheduled phone calls. We can offer you a scheduled phone appointment with your GP to discuss your needs. Please inform our reception staff if your call is urgent. If your medical concern is an emergency, please call an ambulance on 000.
The RACGP does not recommend the use of email for the transfer of patient information. Email is not considered secure as it is not encrypted. We regret we are generally unable to send your results and other clinical information to you by email. If you would like copies of your results we are happy to provide you with a printed copy.
Our Practice is committed to preventative care. We use a computerised reminder system for specific recalls, such as reminders for follow up blood tests and vaccinations.
Your medical file is a confidential document. It is the policy of this Practice to maintain security and confidentiality of personal health information at all times and to ensure that this information is only available to authorised members of staff.
All Government Childhood & Adolescent/Adult Schedule vaccines are available and free of charge for Medicare card holders. Ongoing school program vaccinations are also available.
Travel vaccination advice is available and a script for recommended vaccinations can be organize on the day.
Childhood vaccinations for individuals without Medicare cards will require purchase of the vaccinations prior to the GP appointment, this can be arranged in consultation with your GP.
Anti-social Behaviour Policy
We understand that you may be unwell, stressed or anxious. Our Practice has a zero-tolerance policy towards violence, anti-social behaviour and will not tolerate verbal or physical threats made towards members of the staff or General Practitioners or other patients.

Workcover or MVA consultations
If you wish to have a WorkCover or MVA consultations please inform our reception staff at the time of making the appointment and fill out the relevant form prior to your appointment with the GP.
WorkCover and Motor Vehicle Accident accounts are ultimately the patient’s responsibility. Please ensure your WorkCover claim is submitted and you notify us of your claim number. For Motor Vehicle Accident claims, please identify and advise us of your Compulsory Third Party insurer as soon as possible – details on how to find your insurer can be found at
